Submitting an invoice, W-9, COI, proposal, or photos? Complete this form first, then email the file(s) to management@unitedmanagementholdings.com with your company name and the property in the subject line.

Before Working On Site

Vendor Requirements

To work at a United Management Holdings LLC managed property, vendors must have current documentation on file and written authorization from management.

Certificate of Insurance

A current COI naming United Management Holdings LLC and property ownership as additional insureds, with coverage limits as required by management. Exact additional-insured entity names are provided on request.

License & W-9

A valid California contractor's license for the applicable trade (where required) and a completed W-9 on file before payment can be issued.

Confirmed Scope & Access

A written scope of work confirmed by management before work begins, and site access scheduled and confirmed in advance. Do not begin work without written authorization.
